- 2). Request an application from the organization and fill it out completely before you return it. The application covers requirements needed of all egg donors and asks many questions about your current situation and past. You may need to answer questions about your educational background and medical history.
- 3). Schedule a telephone appointment and answer the questions asked by the interviewer. This appointment serves as an interview and gives the organization the chance to ask questions not covered by the application. The interviewer will also ask questions found on the application and expect to get the same response from you on the phone.
- 4). Attend the medical screening portion of the application process. During the medical screening portion, you're given specific tests to check for genetic disorders and check the current condition of your health. These tests look for certain diseases and disorders, including sexually transmitted diseases. You're also asked to give a complete medical history of your family.
