    • 1). Collect evidence of negligence. Take photographs of any bed sores, poor grooming, unsanitary conditions or any other signs of neglect.

    • 2). Ask other residents if needs are not being met. Write down in a journal any signs of medication errors, wandering of patients or if staff members are slow to respond to requests. Record specific times and dates and the names of staff members on duty.

    • 3). Give your information to the nursing home administrator. Fill out a claim against the nursing home, as suggested by Nursing Home Abuse Resource.

    • 4). Contact Adult Protective Services. You can file a report through the National Center on Elder Abuse. Report your evidence to an advocate.
