    • 1). Record each dependent's name, Social Security number and relationship to you on line 6c. Indicate the total number of dependents, including yourself and your spouse, in the box on line 6d. Multiply the number of dependents per line 6d by $3,650 and record the total on line 42 of form 1040.

    • 2). Calculate the total spent on child care for dependents under the age of 13--or dependents that are disabled during the year--so that you and your spouse could work. Provide the caregiver's name, Social Security number or employer identification number, address and total paid during the year on form 2441. Determine the amount of child care credit you may claim for the year and enter the total on line 48 of form 1040.

    • 3). Determine if you are eligible for the child tax credit. This credit provides a credit of up to $1,000 per child. An eligible child is one who is age 17 or younger at the end of the tax year and is a dependent of the tax filer. Complete the worksheet in the form 1040 instructions and enter your child tax credit on line 51 of form 1040.

    • 4). Complete form 8812 to determine if you are eligible for the additional child tax credit. This credit is a refundable credit. If you do not owe tax, you may still receive a refund if you can take this credit. Record the additional child tax credit on line 65 of form 1040.
