    • 1). Obtain a BCBSTX Provider Record ID on BCBSTX's website or by calling BCBSTX at 972-996-9610 (see "Resources"). The applying provider --- physician or medical professional --- must determine whether a Solo Provider Record ID or Group Provider Record ID is needed. Solo providers include those who do not, or will not, employ other providers, uses his own Social Security number or Tax Identification Number for filing taxes or a provider whose business is not incorporated. Group Provider Record ID numbers are required for providers who practice with other professionals, have a Federal Tax ID number in the name of the corporation or whose billing department is under another incorporated business name.

    • 2). Complete the Blue Cross Blue Shield of Texas Contract/Agreement Online Request form found on the BCBSTX website (see "Resources"). Choose the county the participating provider is located in, then answer all questions on the form. Information necessary to complete the form includes the name of the provider, which BCBSTX programs to join, the location of the practice, hospital affiliations, contact information and supervisor information if necessary.

      To access your local Professional Provider Network office, click in the appropriate county section below to direct your submission. If unsure of a county location, rest the cursor on a county for a pop-up to verify its name. If you have any questions, contact your local Professional Provider Network office.

    • 3). Determine whether the application is for an office-based physician or professional provider or a facility or hospital-base provider. An office-based physician works strictly from a private practice while a facility or hospital-based provider works in a hospital or care giving facility such as a nursing home or assisted living facility.

    • 4). Choose the appropriate credentialing process based on the location of the physician's practice --- office- or hospital-based. Required information for the credentialing phase of the application varies based on the type of provider and location of the practice. Credentialing forms are available for both types of providers on the BCBSTX website (see "Resources").

    • 5). Enroll in the BCBSTX Electronic Funds Transfer (EFT)/Electronic Remittance Advice (ERA) system (see "Resources"). EFT allows providers to receive payment for services electronically through direct deposit, while ERA is a system that provides details on claim payment and remittance information updated within 24 hours after the claim is finalized.

    • 6). Submit the completed application packet, including any supporting documentation, to BCBSTX Provider Administration via fax to 972-996-8230, or mail the packet and supporting documentation to Blue Cross and Blue Shield of Texas, Attn: Provider Administration, P.O. Box 650267, Dallas, TX 75265. BCBSTX prefers all information to be faxed rather than mailed.
