Go to your local DMV office. Bring your driver's license and $10. Complete form MV-15, available at any DMV office. Or, you can print it from the DMV website (see Resources section) and complete it before your visit. The DMV will charge you $10 to search for and print the abstract. You will receive it before you leave the office.

    • 2). Call the DMV and request a copy of your driving abstract. Have your driver's license and a credit card in hand. The DMV answers 518-486-9786 from 8 a.m. to 4 p.m. every weekday except state holidays. In addition to the $10 search fee, the department will charge a $5 "processing fee." You should receive your driving abstract in the mail within two weeks.

    • 3). Request your driving abstract by mail. Get a copy of DMV form MV-15 in a DMV office or by downloading it from the DMV website (see Resources section). Complete it and mail it to NYS Department of Motor Vehicles, 6 Empire State Plaza, Albany, NY 12228. Include a photocopy of your license, and a check or money order for $10 payable to the "Commissioner of Motor Vehicles."

    • 4). Request your driving abstract online. You can use this option only if you have notified the DMV of your most recent change of address, and you must pay with a credit card. If you suspect you have a license suspension because the DMV has lost track of you, this will not work. Within two weeks, the department will mail a copy of your abstract to the most recent address it has for you.
